PROMOTING THE PUBLIC'S HEALTH—Reorienting health policies, linking health promotion and health care. Conclusions drawn from the 2005 EUPHA conference in Graz

The 13th European Conference on Public Health held from 10 to 12 November 2005 in Graz attracted some 900 public health researchers, policy experts and practitioners from all continents. A conference book1 highlighting the outcomes of what turned out to be a highly stimulating scientific meeting is going to be published by mid-November 2006.

The annual EUPHA conferences seem to have a great potential to promote the professional discourse of the international public health community, though their contribution to the development of the field has never been assessed. This book is intended as a contribution to this discourse, as a part of the international public health process, and one that the readers will hopefully also find interesting and informative from the scientific point of view.

Conference books are inherently a difficult genre. The editors of the Graz conference book have attempted to avoid publishing a conference report of merely a series of plenary speeches and oral presentations. Their aim was to capture the general spirit of the conference, summarize the main scientific content and communicate innovative ideas and visions for the future.

The Graz conference addressed three broad questions:

  • Are the determinants of population health a major issue of research, practice, and policies in European public health?
  • What efforts have been taken to build adequate structural and strategic capacities for public health?
  • Is the improvement of population health a meaningful and realistic goal of 21st century public health policy?
From the 5 plenary sessions, the 60 parallel sessions and over 200 posters it was concluded that the European Public Health community shares a fairly strong value and knowledge base related to the determinants of public health which, however, varies markedly between countries. Yet, in most countries there is a serious need to invest in capacity building, foremost in relevant research, education, and training, in order to develop sustained public health infrastructures and promote problem-solving capacities. The Graz conference book shows that European countries need to move towards a Health in all policy,2 if they aim for a more equitable social distribution of health opportunities in the entire population, a better balance of curative care, disease prevention and health promotion—and ultimately a healthier population.

TACKLING THE CHALLENGE OF OBESITY IN EUROPE

Obesity presents Europe with an unprecedented public health challenge that has been underestimated and poorly assessed; governments do not fully accept it as a strategic problem with substantial economic implications. Overweight affects between 30 and 80% of adults in the various countries of the WHO European Region and up to one-third of children. The costs to the health services of treating the resulting ill health—such as type 2 diabetes, certain types of cancer, and cardiovascular disease—are estimated to be up to 6% of total health care expenditure; indirect costs in lost productivity may be as much again.

Preventing obesity is thus an urgent public health goal that should be dealt with through innovative environmental approaches, part of a coherent, progressive, cross-governmental and international strategy, based on short-, medium- and long-term societal changes. Most countries in the WHO European Region have developed nutrition action plans or public health strategies to deal with obesity risk factors, although only a few deal with physical activity. No country has been able to curb the epidemic, yet the obesity epidemic is reversible.

To address the growing challenge posed by the epidemic of obesity to health, economies, and development, WHO is organizing the WHO European Ministerial Conference on Counteracting Obesity (Istanbul, Turkey, 15–17 November 2006). The Conference will be attended by high-level delegations from the health and other relevant sectors in the Member States of the WHO European Region, the European Commission, international organizations, nongovernmental and professional organizations, experts, and the media.

Preparation for the Ministerial Conference has involved government sectors, civil society, experts, and private–public partnerships across the Region in dialogue and consultations, and taken into account a broad range of views, experiences, and suggestions. As a result, several papers have been developed to describe the public health challenge and highlight the areas for action, based on evidence of effectiveness. New tools to support this action have also been developed, such as an outline for a second European action plan on food and nutrition, a framework for promoting physical activity for health, guidelines for action in the school setting and several monitoring tools.

The Conference is expected to adopt a European charter on counteracting obesity, a document containing commitments and precise areas for action that will guide strategies and international cooperation.
